Completing this online form is part of the agency’s formal mechanism through which clients and other stakeholders can express and resolve complaints without interference or retaliation. Within fifteen (15) days of submission, you will receive written notification of the resolution and explanation of any further appeal, rights, or recourse if you remain dissatisfied. Complaints are welcomed in as much as they provide opportunities for performance improvement.
